COVID-19 Re-Opening Procedures

Changes in Patient Intake Procedures:

Before your appointment you will receive a text message with COVID-19 screening questions and an intake form to update your contact information.

Please finish these short forms promptly so that we may review them prior to your appointment.

When you arrive, we will ask you to text us from the parking lot and wait in your car.

Our patient concierge will meet you at your car and escort you into the building.

We will confirm that we received your screening forms, take temperatures on all individuals entering the building and dispense hand sanitizer.

Anyone entering the building will be asked to bring and wear their own mask.

These precautions are in place to limit the risk of transmission to any of our patients and our team.

ONLY the patient should enter the clinic, unless a chaperone/guardian is absolutely necessary. We will be keeping the waiting area empty and the clinical bay free from non-patients for the time being.

The tooth brushing sink will be used for hand hygiene and patients will be asked to rinse with a hydrogen-peroxide mouthwash before being escorted directly back to their chair.

Please brush before your appointment!

Appointment updates and progress reports will be communicated with parents through email, phone and text messaging to limit direct contact.

Please ensure these contact points are updated in the texted intake forms.
